Job Description
The Registered Dietitian is responsible for clinical nutrition services for a skilled care nursing home with high-risk residents and Rehabilitation unit, diet education, and clinical nutritional assessment and services for residents at the (CBRF) Community Based Residential facility and (FDD) Fundamentally Developmentally Disabled facility. This position also assists in managing and directing all 24 hours food service operations and provides clinical nutrition services through the (WIC) Women Infant Children program.
- Conduct in-depth nutritional assessment and develop individualized on-going nutritional plans for healthcare center residents, FDD residents, CBRF residents and rehabilitation residents.
- Develop, review and adapt therapeutic and modified diets to meet the nutritional needs of individual residents in cooperation with health care team, residents and caregivers.
- Communicate effectively with dietary department management team, nursing, social workers, MD, activities, and other professional staff on a daily basis as needed for complex problem solving and decision making to meet and exceed quality care of CCRLC residents, CBRF and WIC program.
- Effectively complete and meet State/Federal computer program assessments/deadlines for Nursing Home and WIC.
- Attend quarterly staffing meetings with healthcare team, residents and family members to meet nutritional needs of residents.
- Assist in directing major project or program updates/changes in a timely and effective manner.
- Manage Flow of WIC clinic including working with staff and clients in an efficient manor at off-site clinics.
- Conduct in-depth nutritional assessment and monitor/evaluate indicators and outcomes data related to nutrition diagnosis/goals/intervention strategies to determine the progress made toward desired outcomes of nutrition care for WIC clients using the Rosie computer program.
- Maintain Registered Dietitian status in state of Wisconsin.
Bachelor's degree in Dietetics and a minimum of two years’ prior experience working in a healthcare setting is required. Individuals must also have a Certified Food Manager Certification recognized by the State of Wisconsin and have the qualification to serve as a Certified Lactation Consultant or Certified Lactation Specialist. Prior experience and knowledge in long-term care regulations
