Job Description

Registered Nurse (RN)Position Summary

The Registered Nurse (RN) is responsible for coordinating and overseeing all aspects of healthcare services for individuals receiving support through the organization. The RN works as a member of a multidisciplinary team to promote health, wellness, recovery, and quality of life through comprehensive nursing care, health education, medication management, and coordination of medical and behavioral health services.

Essential Duties and ResponsibilitiesClinical Care and Health Management
  • Participate as an active member of a multidisciplinary team providing holistic, person-centered, and values-based services.
  • Assess, monitor, and respond to new, ongoing, and emerging health concerns of individuals served.
  • Evaluate medical care needs and identify services that support overall health and wellness.
  • Coordinate care with physicians, prescribers, specialists, and other healthcare providers.
  • Serve as a liaison between individuals, healthcare providers, family members, and community resources to ensure access to appropriate medical care and support services.
  • Recommend health-related goals and interventions for inclusion in individualized recovery plans.
Medication Management
  • Manage the ordering, delivery, storage, administration, and documentation of medications in accordance with organizational policies and program requirements.
  • Administer medications, including intramuscular (IM) injections, as prescribed by authorized medical providers.
  • Monitor vital signs, medication effectiveness, and adverse side effects, communicating findings to the treatment team as appropriate.
  • Collaborate with individuals and treatment teams to improve medication adherence and treatment outcomes.
  • Ensure individuals maintain prescribed medication schedules and receive necessary follow-up care.
Assessment and Documentation
  • Conduct screenings and assessments using validated tools for depression, trauma, substance use, and criminal risk/needs factors.
  • Complete assessments, progress notes, treatment contacts, recovery plans, and related clinical documentation within required timeframes.
  • Utilize collaborative documentation practices and maintain accurate, timely, and confidential records.
  • Ensure compliance with organizational, licensing, accreditation, funding source, and regulatory documentation requirements.
Care Coordination and Recovery Support
  • Provide recovery-oriented nursing services that support evidence-based practices and person-centered treatment planning.
  • Assist individuals in obtaining primary medical care, specialty care, and community resources that promote integrated physical and behavioral health.
  • Work collaboratively with treatment teams to ensure individuals receive annual physical examinations, dental care, and preventive healthcare services.
  • Meet established monthly service delivery and treatment contact requirements.
Education and Training
  • Provide health education to individuals receiving services regarding wellness, medication management, chronic health conditions, and preventive care.
  • Deliver training and consultation to staff regarding health-related issues and best practices in supporting individuals' medical needs.
  • Promote health literacy and informed participation in treatment and recovery planning.
Crisis Intervention
  • Participate in an on-call rotation providing 24-hour emergency and crisis intervention support.
  • Respond effectively to urgent medical, behavioral health, and crisis situations as required.

Other Duties
  • Perform additional duties and responsibilities as assigned by the Assistant Program Director or designated supervisor.
QualificationsRequired
  • Graduate of an accredited professional nursing program (Associate Degree in Nursing, Diploma Nursing Program, or Bachelor of Science in Nursing).
  • Current and unrestricted Registered Nurse (RN) license in the applicable state.
  • Strong clinical assessment, nursing, and care coordination skills.
  • Knowledge of medication administration, health monitoring, and integrated healthcare practices.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Excellent communication, organizational, and documentation skills.
Preferred
  • Bachelor of Science in Nursing (BSN).
  • Experience working in behavioral health, community health, substance use treatment, or recovery-oriented services.
Working Conditions

This position may involve exposure to:

  • Dust, fumes, mists, and gases
  • Extreme heat and cold
  • Wet and humid environments
  • Loud noise levels
  • Confined spaces
  • Mechanical, chemical, electrical, and burn hazards

Work may occur in community-based, residential, outpatient, and office settings. Travel within the service area may be required.

The position requires:

  • Exerting up to 50 pounds of force occasionally
  • Exerting up to 20 pounds of force frequently
  • Exerting up to 10 pounds of force constantly to move or assist with moving objects
