General Purpose:
The Clinical Care Coordinator supports whole-person, patient-centered care by improving health outcomes for MFHC’s patient populations and communities. This role works collaboratively with patients, families, healthcare teams, and community partners to promote healthy behaviors, reduce health disparities, and address barriers impacting overall wellbeing. The Clinical Care Coordinator provides care coordination and population health support for patients at increased risk for poor medical, behavioral, or social outcomes by facilitating care across healthcare settings, connecting patients to community resources, and supporting continuity of care.
Job Qualifications:
- One year of experience in healthcare, care coordination, social services, community health, or a related field is preferred but not required.
- Must demonstrate proficiency in electronic health records and basic care coordination workflows.
- Must possess current valid CPR and/or BLS certification.
- Bilingual fluency in English and Spanish required.
- High School Diploma or GED Equivalent required.
Salary: $23-$28 per hour; DOQ
