Position: Speech-Language Pathologist
Department: Home Health
Supervisor: Clinical Manager of Therapy Services
Are you a compassionate Speech-Language Pathologist who values patient-centered care and enjoys working independently? We are seeking a dedicated Home Health Speech Therapist to join our growing team. In this role, you will help patients regain independence, improve safety, and achieve their functional goals in the place they call home.
As a member of our interdisciplinary team, you will have the opportunity to make a meaningful impact on patients' lives by providing skilled therapy services, caregiver education, and personalized treatment plans that support recovery and quality of life.
What You'll Do- Conduct patient evaluations, assess home environments, and identify communication and swallowing needs.
- Establish measurable goals, administer evidence-based therapeutic interventions, and recommend assistive devices.
- Provide palliative-focused speech therapy for Hospice patients, enhancing quality of life through communication and swallowing support.
- Collaborate with physicians, interdisciplinary teams, patients, and caregivers to coordinate comprehensive care.
- Instruct patients, families, and team members in treatment regimens.
- Document evaluations, clinical notes, and progress timely, meeting Medicare CoPs, PA licensure standards, and agency policies.
- Maintain professional competence through continuing education and best-practice standards.
- Graduate of an accredited Speech Therapy program.
- Current SpeechTherapist license in the state of practice.
- Current CPR certification.
- Valid driver's license, reliable transportation, and current automobile insurance.
- Strong communication, organizational, and clinical assessment skills.
- One or more years of experience as an Speech Therapist.
- Home health experience preferred.
- Experience working with adult and geriatric populations.
- Knowledge of Medicare home health regulations and documentation requirements.
