Position SummaryImperial Health is seeking a compassionate, motivated, and professional Social Worker to join our team in Pasadena, California. The ideal candidate will have 1–2 years of social work experience and a Master of Social Work (MSW) degree.

The Social Worker will provide support, resources, advocacy, and care coordination to patients and their families while collaborating with healthcare professionals to address psychosocial needs and improve overall patient outcomes.

Candidates must currently reside in California and hold a Master of Social Work (MSW) degree.

Essential Duties and Responsibilities

Assess patients' psychosocial needs and identify barriers that may affect their health and well-being.

Provide supportive counseling, education, and appropriate referrals to patients and families.

Assist patients in accessing community resources, healthcare services, financial assistance, housing resources, and other support programs.

Coordinate care and communicate effectively with physicians, nurses, case managers, and other healthcare professionals.

Advocate for patients and help ensure their needs and concerns are addressed.

Develop and maintain appropriate documentation, case notes, assessments, and care plans.

Participate in interdisciplinary care planning and case conferences as needed.

Provide crisis intervention and emotional support when appropriate.

Assist patients and families with navigating healthcare and social service systems.

Maintain confidentiality and comply with applicable healthcare regulations, policies, and professional standards.

Perform other duties as assigned.

QualificationsRequired

Master of Social Work (MSW) degree from an accredited program.

1–2 years of professional social work experience preferred.

Must reside in the state of California.

Strong interpersonal, communication, organizational, and problem-solving skills.

Ability to work effectively with patients, families, healthcare professionals, and community organizations.

Ability to maintain accurate and timely documentation.

Demonstrated compassion, professionalism, and commitment to patient-centered care.

Preferred

Experience working in a healthcare, medical, behavioral health, hospital, or community-based setting.

Knowledge of local and statewide community resources and social service programs.

Experience with care coordination, case management, or patient advocacy.

California social work licensure or associate-level registration, as applicable to the position.
