Job Description

Position Summary:

The Family Care Manager position provides continuity of care, with the goal of permanent placement, for children through a family care management model that includes developing, expanding, accessing, and linking resources in the community to the needs of the child throughout the child’s experience in the system, while documenting progress.

In addition to the duties and requirements listed below, YFA expects employees to be committed to incorporating principles of trauma-informed care in all areas of service delivery. Commitment to these principles is demonstrated through active participation in agency-required training, use of trauma-informed practices, and understanding that trauma-informed environments require cultural sensitivity and an emphasis on a safe and respectful environment for all.

Essential Job Functions:

  • Assess, interpret, and provide care information for children and families.
  • Work as part of a Family Care Team managing children (ages 0-17).
  • Consistently demonstrate positive outlook, patience, respect, and cultural sensitivity when dealing with challenging environments.
  • Contribute and promote a workplace of dignity and respect for all.
  • Serve as a mandatory reporter for child safety.
  • Foster a safe and welcoming environment for youth, families, and colleagues, ensuring emotional and physical security without re-traumatization.
  • Follow a trauma-informed approach, demonstrating sensitivity to individuals’ needs and experiences.
  • Practice self-care and seek support as needed to prevent burnout while providing trauma-informed care.
  • Maintain current knowledge of community resources to recommend.
  • Effectively use constructive feedback and different ways to improve performance and efficiency.
  • Be punctual, ready to work and collaborate effectively in team settings.
  • Take guidance and direction from supervisors and management.
  • Adhere to our organization's core values and mission.
  • Provide case management services including assessment, planning, monitoring, and advocacy.
  • Ensure completion of Medicaid assessments and case/treatment plans for children in foster care.
  • Develop and maintain service/case plans with clear goals, objectives, and timelines.
  • Review and update case plans regularly.
  • Complete and share monthly progress reports.
  • Provide educational support to families about child abuse and neglect.
  • Maintain accurate records in electronic and paper systems.
  • Ensure timely preparation of legal work per Florida Statute.
  • Identify and monitor at-risk families, informing supervisors as needed.
  • Refer children and families to community services for continuity of care.
  • Participate in staff development, team meetings, and client reviews.
  • Build and maintain effective working relationships with team members, supervisors, community providers, and families.
  • Complete additional tasks or projects assigned by management.
  • Protect access and use of protected personal and health information. Confidentiality is a requirement.

Requirements:

Education & Experience:

BSW/BA or MSW/MA in human services required; direct experience in Foster Care/Adoption preferred. Strong verbal and written communication skills, with focus on clear and concise documentation. Ability to use personal vehicle for business when required. Ability to maintain valid Florida State Driver's License and clean Motor Vehicle Record (MVR) with no moving violations. Ability to maintain personal vehicle liability insurance and provide proof of same two times per year and upon request. Ability to demonstrate strong customer service skills and treat all people with respect, patience, and cultural sensitivity. Independent working proficiency in the use of computers and Microsoft software applications, including Outlook, MS Teams, Word, and Excel.

Physical Requirements:

Ability to travel regionally on a regular basis and statewide as needed; ability to bend, lift, and carry 30 pounds.
