JOB SUMMARY: The Outreach Case Manager is Responsible for providing direct assistance and case management to survivors of domestic violence that are seeking services through the Harbor House of Central Florida Outreach Program.
ESSENTIAL FUNCTIONS:
- Works directly with survivors to determine areas of need, set goals, develop a service plan while providing and maintaining accurate client and service records ensuring audit requirements are being met.
- Provides lethality assessments & crisis counseling.
- Provides advocacy, and general case management assistance related to legal, financial, medical, mental health, educational, employment, housing and other social services for survivors and their children while completing necessary records, outcome measures, schedules, time sheets and ensuring survivor information remains confidential.
- Provides linkages and referrals with community service providers to facilitate accomplishment of service plan’s goals. Conducts follow-up meetings with survivors to assess progress toward completing service plan’s goals and identify areas requiring further assistance.
- Facilitates various support and life skills groups.
- Responsible for documentation, collecting and compiling statistical data on clients and services to maintain compliance with established funding sources.
- Work closely with Harbor House INVEST Advocates, EVE Advocates, CPI Advocates, Shelter Case Managers, Court Advocates, IFP Attorneys, and various community agencies to ensure coordination of services.
- Performs other duties as assigned.
MARGINAL DUTIES

KNOWLEDGE, SKILLS AND ABILITIES:
- Knowledge of case management skills.
- Ability to provide effective and immediate crisis intervention for clients
- Ability to facilitate support groups.
- Ability to communicate effectively both orally and in writing.
- Ability to provide leadership in a work and community setting.
- Ability to work with minimum supervision and a high degree of autonomy and productivity.
- Ability to establish and maintain effective working relations with various community agencies, coalitions, and agency staff.
- Excellent communication and advocacy skills.
- Ability to work with a moderate noise level environment.
- Proficiency in basic to intermediate computer skills, including Microsoft Office and database software.
PHYSICAL REQUIREMENTS:
- Frequently standing, sitting, and walking.
- Some stooping, lifting and bending.
- Talking, hearing, seeing, repetitive motions.

MINIMUM REQUIREMENTS:
- Education: Minimum of an undergraduate degree in Social Work, Psychology, Counseling, or related field is preferred.
- Experience: A minimum of one year of case management experience, preferably in a domestic violence program.
- Discretion: Professional experience may be substituted for formal education at the discretion of the Chief Executive Officer.
- Licensing: Valid Florida Driver’s License with a clean record for the past 5 years. Must be 21 years or older to meet agency insurance requirements.
- Skills: Proficiency in Microsoft Office applications (Word, Excel, PowerPoint). Ability to learn organization-specific software, including databases and content management systems. Strong professional writing and communication skills. Ability to manage multiple priorities in a fast-paced environment with attention to detail.
- Training Requirements: Completion of CORE Competency training within 90 days of hire. Additional mandatory training is required on an annual basis per Agency Policies & Procedures.
