MAT COORDINATOR/ LICENSED PRACTICAL NURSE (RN) POSITION SUMMARY:
The Medication Assisted Treatment Licensed Practical Nurse works as part of a team with the MAT Case Manger to manage all aspects of patient care for incarcerated individuals receiving Medication Assisted Treatment for Opioid Use Disorder and deliver a recovery-oriented system of care.
MAT COORDINATOR / LICENSED PRACTICAL NURSE (LPN) MINIMUM REQUIREMENTS:
Must have and maintain current licensure as a Licensed Practical Nurse in Delaware.- Medication Inventory Management
- Initial and ongoing SUD (substance use disorder) assessment and evaluation
- Patient Care Coordination
- Collaborate with community treatment providers and social support services
- Health Promotion
- Comprehensive transitional care and re-entry planning
- Attend meeting and trainings as required
- Dispense MAT Medications
