Job Description

Overview

The Case Worker is a member of the Social Services Department responsible for coordinating casework and therapeutic services for youth placed by the Department of Education (DOE) and Departments of Social Services (DSS) within the RTC's community-based scattered-site program. The role manages an assigned caseload, supports the Social Services Team, and assists with related casework activities across multiple residential locations. Caseloads may vary based on program needs. The Case Worker reports to the Social Work Supervisor and works throughout the RTC's scattered-site locations and community settings.

Key Responsibilities

  • Manage an assigned caseload and maintain accurate, up-to-date case records.
  • Ensure compliance with OCFS, DSS, ACS, CSE, and agency regulations and standards.
  • Complete required documentation, including progress notes, FASPs, permanency reports, incident reports, and court-related reports.
  • Coordinate family engagement activities, including home assessments, family visits, sibling visits, and required contacts.
  • Attend and participate in court hearings, treatment team meetings, FTCs, SPRs, and multidisciplinary meetings.
  • Monitor and track case-related documents, evaluations, entitlements, and legal records.
  • Serve as a third-party reviewer for Service Plan Reviews (SPRs) and Family Treatment Conferences (FTCs).
  • Respond to and document incidents, including Justice Center matters, AWOL events, and critical incidents.
  • Facilitate referrals to community resources and support discharge planning to ensure continuity of care.
  • Attend required trainings and perform other duties as assigned to support program operations and youth outcomes.
  • Other related duties as assigned by supervisor.

Required Qualifications

  • Bachelor’s degree in Human Services, Social Work, Psychology, or a related field preferred.
  • 1-3 years of experience working with adolescents in residential, child welfare, behavioral health, or related human services settings.
  • Valid driver’s license with access to reliable transportation.
  • Strong verbal, written, and interpersonal communication skills.
  • Ability to manage multiple priorities, maintain organization, and work independently with minimal supervision.
  • Bilingual (Spanish) preferred but not required.

Work Environment

This position operates within a 24/7 Residential Treatment Center (RTC) serving youth ages 12–21 across multiple community-based sites. The role requires regular travel to homes, courts, schools, appointments, and RTC locations. Responsibilities include conducting home assessments, supporting family visitation, and attending court hearings and treatment meetings. Flexible hours, including occasional evenings, weekends, and holidays, may be required based on program needs.
