Social Worker-SWR

  1. Acts as liaison with the patient and family and the hospital.
  2. Collaborates with providers, interdisciplinary care team throughout the health system, and community agencies as applicable.
  3. Reviews medical records and interviews patient and/or family as appropriate; assesses and investigates patient's needs and acts accordingly.
  4. Helps to coordinate plans of care, including physical, financial, and psychosocial needs.
  5. Supports the Care Manager with the needs of the patient with complex discharge needs.
  6. Helps to coordinate complex care needs related to women's and children's services including adoption and guardianship.
  7. Demonstrates understanding of the physical and emotional needs of patient and/or family.
  8. Completes tasks as necessary to meet plan of care goals; tasks may include, but are not limited to:
    1. Assistance with arrangement of transportation.
    2. Working with patient on financial assistance.
    3. Making referrals to community resources (DME providers, home health care, skilled nursing facilities, mental health providers, etc.)
    4. Collaborates with agencies such as, but not limited to, APS, CPS, etc.
    5. Assistance with advanced directive as applicable.
  9. Coordinates with Care Manager to confer if interventions need to be modified or adjusted as applicable.
  10. Communicates and collaborates with the OP Care Team when patients transition out of the hospital, if applicable.
  11. Participates in Care Coordination Rounds and Inter-professional Rounds daily, if applicable.
  12. Is knowledgeable regarding pertinent community agencies, institutions, and other resources.
  13. Maintains knowledge of value based programs to ensure appropriate use of payer and patient resources to promote high quality, cost effective care.
  14. Consistently demonstrates ability to balance workload and utilize time effectively as reflected in timely and smooth care transitions.
  15. Utilizes various methods of communication and assesses the effectiveness of each.
  16. Maintains accurate, concise, and timely documentation in the medical record and other department record keeping systems.
  17. Fosters preservation of patient's rights through concept of patient advocacy; possessing understanding and helping people of various backgrounds.
  18. Supervises social work students as requested by the department.
  19. Assumes responsibility for professional growth and development.
  20. Seeks assistance in meeting self-learning needs through in-service, continuing education, self-study, and demonstration.
  21. Shows an understanding of the principles of growth and development over the life span and possesses the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirement relative to his or her age-specific needs.
  22. Demonstrates ability to form positive, collaborative relationships with members of the health care team.
  23. Demonstrates initiative, adaptability, and dependability.
  24. Communicates effectively with others.

Education, Training & Experience

  • Bachelor's Degree or Master's Degree in Social Work or related field (MSW/MS)
  • Strong communication skills with excellent verbal and written skills.
  • Solid knowledge of clinical social work and scope of Indiana State Laws.
  • Social Worker certification by the State of Indiana preferred.
  • Five (5) years of clinical experience is required within the last seven years.
  • Experience in Case Management strongly preferred.
