Concept Rehab is hiring a Full-Time Physical Therapist Assistant (PTA) to provide skilled rehabilitation services in a skilled nursing facility (SNF) in Port Huron, MI. Position: Physical Therapist Assistant (PTA)

Location: Port Huron, Michigan
Employment Type: Full-Time
Setting: Skilled Nursing Facility (SNF)

• Provide physical therapy treatments according to established plans of care and under the supervision of a licensed Physical Therapist.
• Assist patients with therapeutic exercises, functional mobility, transfers, balance, strength, and gait training.
• Monitor patient progress and communicate changes or concerns to the supervising Physical Therapist.
• Complete accurate and timely treatment documentation in accordance with facility and regulatory requirements.
• Collaborate with physical therapists, occupational therapists, nurses, and other healthcare professionals to support patient outcomes.
• Promote patient safety, independence, and functional recovery throughout the rehabilitation process.

• Graduate of an accredited Physical Therapist Assistant program.
• Current Michigan Physical Therapist Assistant (PTA) license.
• Skilled nursing facility (SNF) or long-term care experience preferred.
• Strong clinical, communication, and documentation skills.
• Ability to work collaboratively with an interdisciplinary healthcare team.
• Compassionate approach to patient care and rehabilitation.
