Hope & Help of Central Florida is a mission-driven nonprofit providing HIV healthcare, prevention, case management, and supportive services. Our Ryan White Part D program provides coordinated, family-centered services for women, infants, children, and youth living with HIV across six Central Florida counties: Orange, Osceola, Seminole, Brevard, Lake, and St. Lucie.
The Opportunity
Help women, children, adolescents, and young adults living with HIV stay connected to essential medical care, treatment, and supportive services. This hands-on position is ideal for an experienced case manager who understands the importance of comprehensive assessments, individualized care planning, and consistent follow-through. You'll work closely with healthcare providers, families, and community partners to help clients overcome barriers, remain engaged in treatment, and achieve better health outcomes.
What You’ll Do
•Complete comprehensive assessments of medical, psychosocial, financial, and practical needs; develop and update individualized service plans.
•Coordinate medical appointments, specialty referrals, behavioral health services, transportation, benefits, and other community resources.
•Follow up on referrals and help clients overcome barriers to access, treatment adherence, retention in care, and re-engagement after missed visits.
•Support pregnant clients, mothers, children, adolescents, and young adults, including coordination of appropriate maternal and youth services and transitions in care.
•Collaborate with providers, caregivers when appropriate, community partners, and the multidisciplinary care team while protecting confidentiality.
•Maintain timely case notes, service plans, eligibility documentation, and accurate records in CAREWare and/or Provide or other approved systems.
•Manage an assigned caseload, monitor client progress toward individualized care plan goals, and ensure timely follow-up, documentation, and coordination of services.
What You Bring
•Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, Counseling, Criminal Justice, Public Health, Health Administration, Health Science, or a related field.
•At least six months of direct case management experience.
•Ability to assess complex needs, manage an active caseload, document services accurately, and build respectful relationships with diverse clients.
•Strong follow-through, communication, organizational skills, and a commitment to client-centered care
Experience We Especially Value
•Case management or supportive service experience in Ryan White-funded programs, particularly Part D.
•Knowledge of Ryan White eligibility, documentation, medical case management, and linkage or retention in HIV care.
•Experience using CAREWare and/or Provide.
•Experience serving women, pregnant clients, children, adolescents, young adults, or families affected by HIV.
Location & Travel
Based in Winter Park, with travel up to 40% across Orange, Osceola, Seminole, Brevard, Lake, and St. Lucie counties. Occasional evening or weekend work may be needed for program activities.
Why Join Hope & Help?
Your work can make a direct difference in whether a client connects with care, keeps an appointment, or receives the support their family needs. Join a mission-focused organization where collaboration and meaningful service are central to the work.
