SUMMARY
The Interim Diabetes Quality Coordinator is responsible for overseeing the day-to-day operations of the Diabetes Self-Management Education and Support Service (DSMES) service at all sites. The Interim Diabetes Quality Coordinator ensures that the National Standards (NSDSMES) are met and maintained at all times.


ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
  • Oversees the planning, implementation, and evaluation of the DSMES service. Responsible for maintaining ADA Recognition and participating in the evaluation of the DSMES service’s effectiveness.
  • Plans and implements individual and group curriculum based on knowledge and experience in the areas of diabetes, educational principles, and behavioral change strategies. Incorporates other chronic health problems into diabetes care planning. Maintain appropriate documentation of patient assessment, interventions, and outcomes in the patient’s medical record.
  • Develops with clinical expertise and applicable regulations diabetes management protocols which may include medication management and laboratory monitoring.
  • Coordinates planning of programs for Diabetes Support Group.
  • Coordinates outpatient diabetes education by communicating and collaborating with physicians, nurses, dietitians, public health nurses and other healthcare professionals. Facilitates inpatient hospital referrals for diabetes education as needed.
  • Demonstrates the knowledge of the principles of growth and development through the life span. Possesses the ability to assess data reflective of the patient's status, and interprets the appropriate information needed to identify each patient's needs in relation to age.
  • Assists in the performance evaluation of other health care providers participating in the diabetes education programs.
  • Supports the policies and procedures of the organization and the standards of the Joint Commission as required for full accreditation.


EDUCATION and/or EXPERIENCE

  • At least two years of experience working in a DSMES-ADA Education Recognized Program.
  • Current CDCES. Licensed Registered Nurse.
  • Applied knowledge of adult learning techniques and experience in behavioral change strategies.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
