Job Description
Position Summary
The ID Program Coordinator supports CCI Health Services' Infectious Disease (ID) and HIV programming across all health center sites. This role is responsible for individual patient care coordination, group session support, community outreach, partnership development, patient navigation, and day-to-day program logistics that keep CCI aligned with the Ending the HIV Epidemic (EHE) initiative and best practices in HIV prevention and treatment.
The HIV Program Coordinator works across multidisciplinary care teams and with external community partners to ensure patients newly diagnosed with HIV, those newly initiating PrEP, and those living with HIV receive timely, culturally responsive, and well-coordinated care. This role also serves as the operational backbone of CCI's ID program — tracking data, supporting quality improvement, and ensuring program continuity.
MINIMAL EDUCATIONAL/EXPERIENCE REQUIREMENTS:
Education
· Bachelor's degree required in public health, social work, health education, nursing, or a related field.
· Equivalent combination of education and directly relevant experience will be considered.
Experience
· Minimum two (2) years of professional experience in healthcare, public health, community health, or social services; community health center or FQHC experience preferred.
· Direct experience with HIV programming, infectious disease coordination, sexual and reproductive health, or a closely related area preferred.
· Experience with patient navigation, care coordination, or case management, particularly for underserved or high-need populations.
· Experience facilitating community outreach, health education sessions, or group-based programming.
· Experience working with electronic health records required; eClinicalWorks experience preferred.
· Experience working in multicultural, multilingual environments; bilingual Spanish-English candidates strongly encouraged to apply.
Core Hours are 8am - 4:30pm, M-F
