Physical Therapist - PT Part Time in Altamonte Springs, Florida at Choice Rehab

As a Home Health Physical Therapist, you will:

• Perform and document comprehensive patient evaluations and develop individualized treatment plans.
• Assess patient conditions and determine physical therapy diagnoses prior to intervention.
• Design and implement treatment programs to improve mobility, reduce pain, restore function, and prevent disability.
• Monitor patient progress and modify treatment plans to maximize outcomes.
• Deliver care in accordance with infection control and universal precaution standards.
• Collaborate effectively with patients, families, physicians, and interdisciplinary care teams.
• Ensure timely, accurate documentation and compliance with company standards.
• Promote the mission, values, and patient-first philosophy of Choice Health At Home.

• Graduate of an accredited Physical Therapy program
• Current Physical Therapy license in applicable state
• Minimum 1 year of Home Health experience preferred
• Current CPR Certification
• Valid Driver’s License
• Current Auto Insurance
• Experience with HomeCare HomeBase preferred

• Ability to stand, walk, sit, bend, kneel, squat, crawl, and climb for extended periods
• Ability to lift, carry, push, or pull up to 100 lbs.
