Position Summary
The Physical Therapist Assistant (PTA) provides skilled physical therapy services to patients in their homes under the direction and supervision of a licensed Physical Therapist (PT). The PTA delivers care in accordance with physician orders, the established therapy plan of care, agency policies, and applicable state and federal regulations.
This role supports patient recovery, mobility, safety, and functional independence through hands-on therapy interventions and close collaboration with the interdisciplinary care team.
Reporting Structure
This position reports to the supervising Physical Therapist (PT) and designated clinical leadership. The PTA functions as part of the interdisciplinary care team and provides treatment only under the direction of the licensed PT and within the established plan of care.
Scope of Practice
The PTA performs services within the scope of Florida PTA licensure, Medicare Conditions of Participation, physician orders, and agency policies. The PTA does not perform initial evaluations, establish therapy plans of care, or make independent clinical decisions outside the direction of the supervising PT.
Essential Job Functions
1. Skilled Therapy Services
Provide physical therapy treatments under the direction and supervision of the Physical Therapist (PT)Deliver therapy interventions in accordance with physician orders and the established therapy plan of careAssist patients with therapeutic exercises, gait training, transfer training, balance activities, and mobility restorationSupport fall prevention, strengthening, and safe functional movement in the home setting2. Patient Monitoring & Communication
Observe and report patient response to treatment, progress, and changes in condition to the supervising PTCommunicate patient concerns, safety issues, and therapy barriers promptlyReinforce patient and caregiver education as directed by the PT3. Documentation & Compliance
Complete timely and accurate treatment documentation, progress notes, and visit records in the EMR systemEnsure documentation supports medical necessity and therapy continuityMaintain compliance with Medicare requirements, state regulations, and agency policies4. Team Collaboration
Work closely with the PT, nurses, physicians, and interdisciplinary care team to support continuity of careParticipate in care conferences, discharge planning, and required staff meetingsAttend required in-services, competency reviews, and professional development activitiesQualifications
Graduate of an accredited Physical Therapist Assistant (PTA) program required. Current state PTA licensure or certification in good standing required. Minimum of one (1) year of experience in physical therapy or related clinical setting preferred; home health experience preferred.Current CPR certification requiredValid driver’s license, reliable transportation, and auto insuranceAbility to pass background screening and all compliance requirementsSkills We Value
