Job Title
Clinic Licensed Practical Nurse (LPN)
Job Summary
Participates in the planning, delivery, and evaluation of quality patient care within the guidelines and policies of the department. Responsible for meeting established standards of practice and patient care, supplies and equipment, and for coordinating teamwork with all members of the health care team.
Compensation
We are committed to providing wage ranges for all open positions. Please note that the specific compensation for this role will be determined based on experience, education and internal equity considerations. The hourly wage range for this position is $23.71–$33.47. This range reflects the base salary for this position.
Key Accountabilities
  • Provides direct, individualized nursing care within the LPN’s scope of practice.
    • Prepares and gathers all medical information for scheduled patients.
    • Prepares charts for appointments and reviews pertinent information for the appointment.
    • Conducts a focused assessment of the health status of the patient through the collection and comparison of data to normal findings and the patients’ current health status.
    • Implements interventions that are delegated, ordered, or prescribed by the provider, including but not limited to electrocardiograms, Holter monitors, applying splints and braces, wound dressing changes, suture removal, and staple removal.
    • Assists in the evaluation of responses to interventions, and reports on changes and responses to nursing interventions to the RN Care Coordinator or provider.
    • Provides health care information to individual patients.
    • Prepares, administers and records medications in accordance with the LPN Medication Administration policy.
    • Prepares laboratory specimens, performs urinalysis, glucose testing, and drug test collection in accordance with hospital lab policies.
    • Monitors providers’ orders to ensure orders are completed.
    • Assist provider as needed.
  • Assigns and monitors nursing tasks or activities to unlicensed assistive personnel.
  • Equipment and supplies
    • Ensures that equipment and supplies are available and adequately stocked for patient care. Prepares supply list and assists the Patient Care Supervisor in ordering supplies and equipment for clinical use.
    • Properly cleans and sterilizes instruments and equipment.
  • Communication
    • Advocates for the best interests of the patients.
    • Collaborates and communicates with the health care team to ensure the quality and continuity of patient care.
    • Communicates pertinent information to providers and healthcare teams.
    • Informs medical/nursing team when a patient has urgent needs and/or a changing condition.
    • Relays test results accurately and in accordance with established procedures.
    • Accurately communicate and records information in a timely manner.
  • Provides input into the development of department policies and procedures.
Required Qualifications
  • Current LPN License in MN
  • Current BLS must be enrolled in NH+C RQI program and complete the requirements within 90 days of enrollment. Must remain current (complete quarterly assignments prior to the end of each calendar quarter).
  • Ability to learn and use electronic health records and other computer programs
  • Must be able to speak, read, and write English proficiently to ensure safe and effective communication in a healthcare environment.
Preferred Qualifications
  • One year of clinical practice as an LPN
