Responsibilities

Perform in-home PT evaluations and treatments

Develop and adjust individualized care plans

Educate patients and caregivers

Document care in EMR (WellSky or HCHB preferred)

Collaborate with interdisciplinary team

Supervise PTAs

Ensure regulatory compliance

Qualifications

Active Virginia PT license

Accredited PT program graduate

3+ years experience (home health or SNF preferred)

EMR experience; OASIS a plus

Valid driver’s license & reliable transportation
