FULL TIME
Annual Salary: $65,000-$70,000
Flexible Schedule: Days, evenings, nights, and weekends at a variety of locations as determined by client healthcare needs and schedules.
Location(s): Serving Clients in the Homes in St. Charles and St. Louis County

QUALIFICATIONS
Must possess a current unencumbered Missouri RN license.
Experience in supporting individuals with Intellectual and Developmental Disabilities preferred, but not required.
Ability to employ active listening and critical thinking to assist and support employees in following healthcare plans for clients.
Ability to enter all client homes.
Ability to read, comprehend, write and speak effectively in English
Proficiency with Microsoft Office applications
Reliable transportation, current vehicle insurance and valid driver’s license for travel to client homes
Excellent organizational and time management skills
Effective communication with staff, clients and families.
Experience with DMH's HRST system preferred.

CORE RESPONSIBILITIES
  1. Make recommendations regarding client safety and hygiene to the Director of Health Services and client support team.
  2. Maintain nursing files and schedules for their assigned caseload.
  3. Assist the Director of Health Services with gathering medical information from program staff and clients for the health assessment of clients.
  4. Assess the physical and psychological dimensions of the clients on caseload monthly.
  5. Review the client Physicians Order(s) and Medicine Administration Records, transcribe and implement physician and nursing orders accurately as received and follow up with the client’s program service manager and the Director of Health Services to support implementation.
  6. Prepare and submit RN Monthly Summary reports to the Director of Health Services and client team including client status, plan of care, any special occurrences and/or significant information as contained in the RN Monthly Summary form.
  7. Provide for confidentiality regarding all matters pertaining to clients and families as per HIPAA guidelines.
  8. Identify staff training needs for assigned clients and facilitate training as directed by the Director of Health Services.
  9. Assist with the maintenance of complete and accurate client health records

LOCATION
The main office is located in St. Charles, MO.
Client homes are throughout the St. Louis and St. Charles County area
