Sagamok Anishnawbek is seeking a dedicated and experienced Registered Nurse – Clinical Lead to provide leadership and clinical oversight within the Community Wellness Department.
The Clinical Lead plays a key role in supporting community health by supervising nursing staff, coordinating home and community care services, and ensuring the delivery of high-quality, culturally safe healthcare. This role integrates clinical expertise, leadership, and community engagement to support the health and wellness of Sagamok members.
The successful candidate will work collaboratively with internal teams, community partners, and regional health organizations to ensure seamless healthcare services for the community.
Key ResponsibilitiesClinical and Community-Based Care- Conduct comprehensive health assessments, treatments, referrals, and follow-ups in both clinical and home settings.
- Provide immunizations, chronic disease management, maternal and child health services, and urgent care coordination.
- Support clients and families in navigating external healthcare services and specialists.
- Develop and manage chronic disease care plans with clients and healthcare providers.
- Ensure culturally safe, trauma-informed care aligned with Sagamok values and traditions.
- Maintain accurate documentation in accordance with clinical, privacy, and regulatory standards.
- Plan, implement, and evaluate community health promotion and disease prevention programs.
- Support community health emergency preparedness and outbreak response efforts.
- Conduct communicable disease surveillance and management.
- Lead vaccination clinics and health initiatives addressing priority health concerns.
- Deliver community-based health education on topics such as:
- Diabetes
- Mental health and wellness
- Substance use
- Nutrition
- Sexual and reproductive health
- Participate in community outreach activities, events, and wellness initiatives.
- Provide leadership, supervision, and mentorship to nursing staff.
- Assist with the development and implementation of clinical protocols and policies.
- Conduct staff performance reviews and evaluations.
- Support continuous quality improvement in patient care and service delivery.
- Oversee home and community care services to ensure accessibility and cultural relevance.
- Collaborate with interdisciplinary teams to enhance patient and family-centered care.
- Maintain partnerships with community stakeholders and external service providers.
- Identify training needs and coordinate professional development opportunities for nursing staff.
- Collaborate with Human Resources to support ongoing clinical education and skills development.
- Ensure compliance with healthcare regulations, licensing standards, and best practices.
- Build and maintain relationships with external healthcare organizations including:
- Maamwesying North Shore Community Health Services
- Maamwesying Ontario Health Team
- Public Health
- Regional hospitals and healthcare providers
- Represent Sagamok Anishnawbek in healthcare partnerships and regional health initiatives.
- Facilitate coordination and communication between community services and external providers.
- Registered Nurse (RN) with a valid license to practice in Ontario.
- Leadership or management experience in a clinical setting.
- Experience in community health, home care coordination, and healthcare systems navigation.
- Experience supporting nursing education and professional development.
- Strong understanding of Indigenous healthcare practices and community health programming.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work collaboratively within interdisciplinary teams and community partnerships.
- Knowledge of First Nations health systems and Indigenous health policies.
- Certification in Public Health Nursing, Advanced Nursing Practice, or Leadership.
- NSWOCC certification.
- Knowledge of Ojibwe or other Indigenous languages.

This role is primarily based within the Community Wellness Department at Sagamok Anishnawbek, with responsibilities that may include clinic work, community outreach, and home visits.
The position requires collaboration with healthcare teams, community members, and external health organizations, and may involve occasional travel and flexible hours depending on community needs.
