About This Position
PRIMARY FUNCTION: Pediatric Associates is a patient focused practice. Our goal is to offer the best experience for our patients at all times. The primary functions of the Care Navigator is to ensure timely patient follow up and coordinate transitions of care from hospitals and health care systems to the primary care provider.
ESSENTIAL FUNCTIONS OF THE JOB: (This list may not include all of the duties that may be assigned.)

Contacting patients to schedule appropriate visits to meet specific patient needs

Perform scheduling of patient appointments using clinical priorities

Perform warms calls to newborn patients welcoming them to the practice, introducing them to our services and scheduling follow up appointments

Coordinate the retrieval and upload of discharge summaries, consult notes, and additional information pertinent to patient discharges to the patient’s record

Register new patient by gathering pertinent information and entering it into multiple electronic health records

Enter patient insurances and verifying patient insurance coverage is current

Schedule and coordinate appointments in the practice management-scheduling module.

Protects patient information under HIPAA laws.

Receive and convey messages electronically via multiple sources.

Working the list of patients in need of outreach and successfully scheduling appointments to close gap in care

Reconciling and reviewing patent charts to ensure they are accurate at time of visit
TYPICAL WORKING CONDITIONS: Working in an office environment. Involves frequent telephone contact interaction. May require sitting for long periods; also stooping, bending and stretching for files and supplies. Occasionally lifting files, boxes weighing up to 25 pounds. Requires manual dexterity sufficient to operate a keyboard, type at 35 wpm, operate, copier, and such other office equipment as necessary. Must be able to communicate clearly. It is necessary to view and type on computer screens for long periods and to work in a high volume environment.

Must have a home work-space conducive to HIPAA standards (will be HIPAA trained).

Good visual acuity through normal or corrected vision

Good hearing acuity.

Ability to be able to navigate various applications and function in a fast-paced environment.
PERFORMANCE REQUIREMENTS:
Adhere to all organizational information security policies and protect all sensitive information including but not limited to ePHI and PHI in accordance with organizational policy, Federal, State, and local regulations.
Remote outreach position (non-patient facing physically)
Education: High School Diploma or equivalent
Preferred: Bachelor’s degree
Required: High School Diploma or Graduation from an accredited Nursing Program
Licensure/Certification: Medical Assistant diploma/ certificate
Experience:
Minimum of 1-3 years of previous healthcare experience required
Knowledge, Skills & Abilities:

Must possess strong communication skills and phone presentation in interactions with patients and staff

Must be able to utilize appropriate customer service skills in all encounters with patients, parent and/or guardians, professional contacts and employees.

Ability to make large volume of quality outbound calls a day

Must be proficient with computers and computer applications (Electronic Health Records, spreadsheets, word documents, electronic mail, and additional systems as needed).

Must be able to prioritize and organize workload, work with minimal supervision, organize and complete tasks accurately and in a timely manner.

Demonstrated skills in effective written and verbal communication.

Must be able to follow a defined list of tasks and follow the established procedures completing them in a timely fashion.
