Job Description: Speech Language Pathologist in Home Health

We are seeking a highly skilled and compassionate Speech Language Pathologist to join our team in providing exceptional care to our patients in their homes. As a Speech Language Pathologist in Home Health, you will be responsible for assessing, diagnosing, and treating patients with communication and swallowing disorders.

Responsibilities:

- Conduct comprehensive assessments of patients' communication and swallowing abilities

- Develop individualized treatment plans based on assessment results

- Provide therapy to patients to improve their communication and swallowing abilities

- Educate patients and their families on communication and swallowing strategies

- Collaborate with other healthcare professionals to ensure coordinated care for patients

- Maintain accurate and timely documentation of patient progress and treatment plans

Requirements:

- Master's degree in Speech Language Pathology from an accredited program

- Current state license to practice as a Speech Language Pathologist

- Certificate of Clinical Competence (CCC) from the American Speech-Language-Hearing Association (ASHA)

- Minimum of 1 year of experience in a home health setting

- Strong communication and interpersonal skills

- Ability to work independently and as part of a team

- Valid driver's license and reliable transportation
