About This Position
1) This is a Hybrid Position (3 days on site and 2 days' work from home). This could change at any time based on business needs 2) Review patients who are high cost/high utilizers 3) Reduce ER visits/readmissions/long length of stay 4) Assist member with any SDOH issues that may arise 5) Streamline transition in and out of the hospital 6) Schedule follow-up appointments after inpatient discharge and/or ER visits 7) Develop effective working relationships with PCPs, Specialists, care team members, Administrative Leadership and Physician Champion 8) Address all chronic conditions assessments 9) Develop and maintain excellent working knowledge of disease management/care manager/health coaching skills and systems; stays abreast of current/upcoming enhancements to this field of knowledge; maintain excellent knowledge of disease state assigned 10) Attend trainings that are required through this program 11) Must be proficient in Excel 12) Excellent computer skills
MINIMUM Education:
- Associate’s or Bachelor’s degree in Nursing required; if incumbent has an Associate’s, Bachelor’s in Healthcare related field required.
- Master’s Degree strongly preferred.
MINIMUM WORK EXPERIENCE:
- Minimum five 3–5 years experience in a related clinical nursing position. Prior experience in disease management/care management/health coaching is highly desirable.
KNOWLEDGE & SKILLS:
- Demonstrated knowledge and experience in management of disease processes
- Demonstrated knowledge of disease management/care management/health coach processes
- Demonstrated ability to develop and employ effective patient teach back methods
- Demonstrated knowledge of facilitating and coordinating patient care plans
- Excellent oral, written, and interpersonal communication skills. Ability to work independently and also as a team with a wide variety of individuals; Excellent organizational skills
- High degree of creativity in problem-solving; ability and patience to navigate and impact complex systems of care.
- Proficient computer skills
- Willingness to work some weekends
REQUIRED LICENSES, Certificates, Registrations:
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a current Illinois license required; Disease management certification desired.
