🎯 About the Role
We’re looking for a dedicated Clinical Dietitian to join our team and help us provide Medical Nutrition Therapy to patients in a variety of settings using the Nutrition Care Process and principles of evidence-based practice to appropriately assess and re-assess patients and develop individualized nutrition treatment plans.
- Assessment and Implementation: Using The Nutrition Care Process, conducts thorough nutrition assessments and re-assessments, identifies appropriate nutrition diagnoses and interventions to address these diagnoses and monitors and evaluates the effectiveness of the interventions, revising as necessary. Attention is paid to patient-centered and evidence-based care at all times.
- Evaluation: The effectiveness of interventions and specific patient-focused outcomes are measured and evaluated. They are revised as appropriate to meet the unique needs of each patient.
- Communication: Individualized plans of care are developed and documented. Referring providers are communicated with to coordinate patient care. Effective and appropriate communication with patients, family members, staff, professional colleagues and community members in a variety of settings is standard practice.
- Education: Patient education on a wide range of nutrition topics is provided in both individual and group settings. Acts as a nutrition resource for colleagues, patients, other departments and the Greater Saratoga community.
- Resources: Seeks out educational opportunities to stay current and competent in all areas of work responsibilities, including age specific competency. Utilizes appropriate resources to streamline work processes and solve problems as they arise.
- Community: Develops and facilitates community health education programs and exhibits on a variety of topics. Promotes services with potential and current customer groups.
What You Bring
- Bachelor of Science degree in Human Nutrition and Dietetics.
- Must be a Registered Dietitian, with current registration through The Academy of Nutrition and Dietetics
- CDN Certification required.
- Knowledge of Nutrition Care Process encouraged.
