Licensed Clinical Social Worker at PRIMARY CARE OF CENTRAL FLORIDA INC – Orlando, Florida

Our company was founded in 2005 and serves the Central Florida geriatric population. We are a large practice with over 25 practitioners that serve the skilled nursing facilities, assisted living facilities and our homebound patients in the comfort of their home in 14 counties. We are committed to providing excellent care to our patients and buildings while reducing readmissions into the hospital.

The LCSW will provide individual therapy to patients experiencing a wide range of mental health challenges. You will utilize evidence-based practices to assess patients' needs, develop treatment plans, and provide therapeutic interventions. This position offers a unique opportunity to work in a collaborative environment with a team of family medicine and mental health providers to deliver integrated care to patients.

Responsibilities:

Conduct comprehensive mental health assessments to evaluate patients' needs.

Follow individualized treatment plans created by the Psychiatrist or mental health providers.

Provide individual therapy to address a variety of mental health concerns, including anxiety, depression, stress management, dementia and relationship issues.

Utilize evidence-based therapeutic techniques, such as cognitive behavioral therapy (CBT) and motivational interviewing.

Collaborate with family medicine physicians and other healthcare providers to ensure integrated care for patients.

Document therapy sessions and progress notes in accordance with professional standards and EHR systems.

Participate in case management activities, including coordinating referrals and providing resources to patients.

Maintain a safe and therapeutic environment for all patients.

Participate in ongoing professional development activities to stay up to date on the latest mental health treatment approaches.

Conduct therapy sessions via telehealth, in-clinic, at assisted living facilities, and occasionally in patients' homes (with proper approvals).

Qualifications:

Master's degree in Social Work (MSW) from an accredited program.

Licensed Clinical Social Worker (LCSW) license in the state of Florida.

Minimum 1 year of experience providing clinical social work services in a healthcare setting.

Experience with telehealth platforms preferred.

Strong understanding of evidence-based mental health treatment approaches, particularly CBT and motivational interviewing.

Excellent communication, interpersonal, and collaboration skills.

Ability to work independently and as part of a team.

Strong clinical documentation skills.

Compassionate and patient-centered approach to care.

Culturally competent and able to work effectively with a diverse patient population.
