Job Description

POSITION OVERVIEW:

The LMSW is responsible for coordinating all services related to the social and mental health needs of the clients including: assessment of needs, diagnoses, psychotherapy services, referral services as warranted in addition to connecting clients with social service agencies/departments such as housing, health homes and behavioral health.

KEY ESSENTIAL FUNCTIONS:

  • Under the supervision of the LCSW, provide individual, family, and group therapeutic sessions as needed
  • Under the supervision of the LCSW, complete psychosocial assessments and corresponding treatment plans
  • Under the supervision of the LCSW, provide crisis management as needed
  • Maintain up-to-date client records in accordance to organizational guidelines
  • Submit paperwork within policy timeline that meets insurer guidelines for proper billing
  • Organize and facilitate consistent interdisciplinary case conferences
  • Participate in staff, case management, and organizational meetings as requested
  • Gather patient data as necessary to report to supervisor
  • Work closely with Senior Director of Behavioral Health in Primary Care on special projects.
  • Perform additional relevant duties as requested by supervisor.

REQUIREMENTS:

  • Master’s Degree in Social Work or an equivalent related field required.
  • Current and unrestricted LMSW– NYS Licensure
  • Minimum of two (2) of previous relevant experience in a Mental or Behavioral Health environment required.
  • Obtain prior to hire date:
    • Mandated reporter (2 hours)

  • Obtain within six months of hire:
  • Supporting Recovery with Medications Addictions Treatment (MAT)
  • Screening, Brief Intervention and Referral to Treatment
  • CASAC Canon of Ethics (6 hours)
  • Confidentiality related to 42CFR (3 Hours)
  • Ability to use business acumen to exercise good judgment and decision-making.
  • Ability to deal with sensitive issues with diplomacy and discretion.
  • Must be able to multitask with strong organizational skills.
  • Excellent interpersonal skills and able to communicate both verbally and in written form.
  • Commitment to cultural diversity and sensitivity.
  • Ability to maintain a professional demeanor under pressure and operate with a keen sense of urgency and commitment to quality.
  • Bilingual (Spanish/ English) is strongly preferred
