We’re seeking a compassionate and organized Patient Care Coordinator in our Free Health Care Center in Downtown Pittsburgh.

The Patient Care Coordinator serves as a vital link between patients, healthcare providers, community resources, and clinic staff. Working under the direction of licensed healthcare providers, the Patient Care Coordinator assists with patient intake, basic clinical support services, patient navigation, and care coordination to promote efficient, high-quality care within the Free Health Care Center.

Welcome patients and assist with the intake process.

  • Coordinate referrals to specialty care and community resources.
  • Assist patients in scheduling follow-up appointments.
  • Assist with prescription facilitation according to pharmaceutical guidelines and policies.
  • Reinforce provider instructions using approved educational materials.
  • Support patient navigation throughout the continuum of care.
  • Complete in-house Medical Assistant training by licensed health care provider.
  • Escort and room patients.
  • Obtain and document patient vital signs.
  • Document assigned activities accurately in the electronic health record.
  • Communicate pertinent patient information to licensed providers promptly.
  • Maintain patient confidentiality in accordance with HIPAA regulations.
  • Participate in team meetings, case discussions, and interdisciplinary care planning.
  • Support volunteer providers, students, and trainees as appropriate.
Requirements:
  • A high school diploma or equivalent is required.
  • One year of experience in healthcare, human services, or customer service is required.
  • Current BLS/CPR certification (or ability to obtain within 90 days of hire) is required.
  • The ability to complete the FHCC Clinical Support Competency Program is required.
  • Strong interpersonal and communication skills are required.
  • The ability to work effectively with diverse and underserved populations is required.
  • Basic computer proficiency is required.
  • Act 33 and 34 Clearances are required.
  • Candidates must not be listed on the U.S. Department of Health and Human Services Office of Inspector General (OIG) List of Excluded Individuals and Entities (LEIE) or any applicable federal or state exclusion database.
