About the Position

Hope & Help is seeking a compassionate, organized, and client-focused Referral Specialist to join our Ryan White Case Management team.

The Referral Specialist helps individuals access and remain connected to HIV medical care and essential support services. This position conducts Ryan White eligibility screening, coordinates referrals, assists clients in navigating available benefits and services, and follows referrals through completion. The role plays an important part in reducing barriers to care and supporting linkage to care, retention in care, and viral suppression.

What You’ll Do
-Conduct initial screenings for Ryan White program eligibility and assist with enrollment and recertification.
-Verify required documentation, including insurance, income, residency, and HIV status.
-Connect clients with HIV medical care, medical case management, housing, food, transportation, behavioral health, and other support services.
-Track referrals and follow up to confirm clients successfully connect with needed services.
-Help clients understand and navigate the Ryan White system, Medicaid, ADAP, --PAPs, insurance, and other available resources.
-Identify and help reduce barriers involving transportation, documentation, insurance, and access to care.
-Maintain accurate and timely documentation of referrals, client contacts, eligibility, and outcomes.
-Collaborate with case managers, clinical teams, outreach/linkage staff, and community partners to coordinate client care.

Required:
-Associate’s degree in social work, psychology, human services, public health, or a closely related field.
-At least one year of experience in health or social services, patient navigation, or a closely related role.
-Strong communication, organization, documentation, and client-service skills.
-Ability to manage competing priorities, deadlines, and client needs.
-Valid Florida driver’s license with a clean driving record.
-Ability to work effectively and respectfully with individuals from diverse backgrounds.

Preferred:
-Bachelor’s degree.
-Previous HIV/AIDS or Ryan White program experience.
-Knowledge of HIV-related services, benefits, insurance, and community resources.
-Experience with EHR/HMIS systems or similar databases.
-Bilingual skills.
