  • Utilize home care management software to oversee daily service operations.
  • Identify and resolve service discrepancies and documentation concerns.
  • Assist with incident reporting, quality assurance, and operational follow-up.
  • Track unstaffed hours, caregiver availability, and outstanding service needs.
  • Maintain organized, accurate, and timely operational records.

Relationship Development & Growth

  • Build and maintain positive relationships with Medicaid referral sources, VA contacts, and community partners.
  • Support client referrals, caregiver recruitment, and service expansion.
  • Identify opportunities to improve service delivery and strengthen client and caregiver retention.
Requirements:

Required Qualifications

  • Minimum of 1 year of direct, hands-on experience working with Medicaid-funded home care services, Medicaid waiver programs, or Home and Community-Based Services (HCBS).

  • Proficiency with computers, electronic documentation, and scheduling systems.

Highly Preferred Qualifications

  • 2 or more years of direct experience coordinating Medicaid-funded home care services or waiver programs.
  • 1–2 years of direct experience with Veterans Affairs (VA)-funded home care services, including VA Community Care, Homemaker/Home Health Aide, or Veteran-Directed Care programs.
