Job Description

Social Worker- Hospice

Visiting Nurse Health System (Visiting Nurse) is hiring for a dynamic with a high EQ Hospice, MSW, for our Members in Fulton County. We seek a Social Worker, MSW, who provides case management activities necessary to meet the needs of clients assigned to the consolidated care team. The Hospice Medical Social Worker provides social, emotional, and psychological services/counseling to terminally ill patients and their bereaved families, enabling the patient to maintain independence and remain at home. The Hospice Medical Social Worker will facilitate alternative care placement requested by patient/family to an appropriate facility.

In this position you will perform...

Coordination of Services:

  • Conduct comprehensive psychosocial assessments of hospice patients and families.
  • Provide counseling, emotional support, crisis intervention, and bereavement services.
  • Develop and implement individualized care plans to address social, emotional, financial, and environmental needs.
  • Connect patients and families with community resources, financial assistance, and government benefit programs.
  • Advocate for patients and families while serving as a liaison between the agency and community resources.
  • Collaborate with the interdisciplinary team to coordinate patient care and participate in care planning meetings.
  • Educate patients, families, and staff on hospice services, coping strategies, boundaries, and available support resources.
  • Maintain accurate, timely, and compliant documentation, assessments, and required regulatory paperwork.
  • Participate in the agency's on-call rotation and respond to patient and family needs as required.
  • Deliver compassionate, culturally sensitive, patient- and family-centered care while adhering to agency policies and regulatory standards.
  • Develops appropriate care plans in consultation with the client, client’s family, and service providers.

  • Ensures that documentation is consistent with the format required by depart cognitive standards (i.e., progress notes reflect care plans.)
  • Maintains confidential case records on all clients.
  • Demonstrates the ability to follow through in a thorough and timely manner on tasks assigned by management team and requests made by patients/families, referral sources, and community.
  • Documents appropriate follow up on client needs whether related to Hospice services or other community resource needs.

Do you have a....

  • Master’s degree in Social Work, Sociology, Psychology, or a related field
  • Two years of experience in case management in a health care field
  • Two years of experience with Medicare, Medicaid and other Funding Source
  • Reliable transportation including a valid driver's license and proof of vehicle insurance
