The Redding Rancheria is seeking a Patient Wellness Coordinator. The preferred schedule for this position is Monday - Friday 8:00am to 5:00pm.
The listed wage reflects the entry-level rate for this position. Compensation may be adjusted based on experience, education, and qualifications
JOB SUMMARY:
Oversight of patient’s wellness projects ensuring that conceptualization, design, implementation, monitoring and project activities are carried out. With the goal and purpose of cultivating interest and encouraging patients to create and maintain a healthy lifestyle.
REQUIREMENTS/MINIMUM QUALIFICATIONS:
- Bachelor’s degree in Public Health, Health Education, or a related field or two years of experience working in related health field.
- Experience with program management including skills in program coordination.
- Knowledge of principles of evaluating program effectiveness and quality assurance
- Possess intermediate level knowledge of personal computers and software including Microsoft Windows, Microsoft Excel, Word, PowerPoint, and Publisher in addition to proficiency with Internet Explorer.
- Must pass pre-employment drug screening.
- Indian preference.
ESSENTIAL JOB DUTIES:
- Lead the development and execution of effective patient wellness programs, activities, and campaigns that promote physical, mental, and emotional well-being for patients.
- Design, compile, and market wellness activities that support all levels of health.
- Develop and maintain a plan to distribute wellness information (clinic goals, activities, incentives, etc.) for the purpose of relaying information and promoting the wellness program.
- Prepare regular activity and participation reports.
- Monitor the effectiveness of wellness initiatives through data tracking, patient feedback, and measurable health outcomes.
- Act as a liaison between patients and providers to support their wellness journey.
- Collaborate with healthcare providers, behavioral health staff, fitness and nutrition professionals, and community partners to coordinate holistic patient care and support.
- Coordinate a variety of activities to encourage all patients to participate.
- Incorporate Quality and GPRA measures within the wellness program.
- Assist with the development and implementation of additional pilot programs.
- Coordinate and schedule patient wellness classes.
- Serve as the primary point of contact for wellness programming, answering patient questions and providing encouragement and resources.
- Conduct educational sessions, wellness workshops, and group events to cultivate interest in healthy living.
- Identify and address barriers to patient participation in wellness programs.
- Maintain up-to-date knowledge of evidence-based health promotion strategies.
- Ensure culturally appropriate, accessible, and inclusive wellness practices for diverse patient populations.
- Advocate for the inclusion of traditional healing modalities alongside clinical services.
