Job Description
Case Manager (Mental Health )
The Case Manager serves as a key member of Arundel Lodge’s Residential Rehabilitation Program, supporting residents in their recovery and overall well-being. This position is responsible for coordinating entitlements, linking individuals to community-based resources, assisting with discharge planning, and promoting independence through person-centered care. The Case Manager acts as an advocate for clients throughout their journey of recovery from mental illness, substance use, and co-occurring conditions.Essential Duties and Responsibilities
• Complete and maintain accurate benefit entitlement applications and renewals (SSI, SSDI, Medicaid, SNAP, etc.).
• Verify Representative Payee status, track residential fees, and resolve discrepancies in collaboration with the business office.
• Assist clients with health insurance enrollment and re-certification (including Medicaid Managed Care and Medicare plans).
• Participate in interdisciplinary team meetings and contribute to the development and implementation of Individual Recovery Plans (IRPs).
• Provide case management services that link residents to community resources such as outpatient therapy, employment services, educational programs, and primary care providers.
Experience:
• Minimum one (1) year of experience in mental health, residential, or psychosocial rehabilitation settings required.
• Experience with discharge planning, community linkage, and entitlement programs strongly preferred.
Knowledge, Skills, and Abilities
• Knowledge of behavioral health systems, benefits programs, and local community resources.
• Strong organizational, communication, and problem-solving skills.
• Ability to maintain professional boundaries while building supportive relationships with residents.
• Proficiency in Microsoft Office Suite and electronic health record systems.
• Culturally competent and able to engage with individuals from diverse backgrounds.
Requirements
• Valid driver’s license with acceptable driving record.
• Satisfactory background check and drug screening.
• Maintain compliance with mandatory trainings and certifications (e.g., CPR/First Aid).

Work Environment
• Work performed in a residential treatment setting and community environments.
• Requires regular interaction with clients, staff, and external agencies.
• Occasional local travel for client appointments and service coordination.

Conduct intake assessments and gather documentation to determine program eligibility and service needs.
• Complete and maintain accurate benefit entitlement applications and renewals (SSI, SSDI, Medicaid, SNAP, etc.).
• Verify Representative Payee status, track residential fees, and resolve discrepancies in collaboration with the business office.
• Assist clients with health insurance enrollment and re-certification (including Medicaid Managed Care and Medicare plans).

• Bachelor’s degree in Human Services, Psychology, Social Work, Rehabilitation, or a related field preferred.
• Equivalent experience may be substituted at the discretion of the Residential Director.
