The primary focus of the Registered Practical Nurse (RPN) is to perform physical assessments, health education and counselling according to the College of Nurses of Ontario (CNO) standards of practice for RPNs, including the following:
- Assist with triaging primary care appointments; history taking, and other clinical activities according to the CNO standards of nursing practice
- Conduct clinical procedures with clients, within scope of practice and in accordance with CNO Standards and organizational procedures, (i.e. suture removal, dressing changes, injections, immunizations, allergy treatments, venipuncture, urine testing)
- Uses a trauma informed approach to support clients and their families
- Participates in the development and implementation of medical directives, as appropriate and ensures related education and skills are up-to-date related to medical directives
- Assists with systems navigation and client advocacy among external health care partners, as appropriate
- Actively participates in assisting with preventative care/cancer screening activities
- Participate in public speaking in various venues and community partnerships
- Ensure clinical and office areas, resources, materials, processes and equipment are maintained in communication and collaboration with other team members
- Provide clinical assessments addressing the physical and emotional needs of clients
- Contribute to chart documentation, participate in chart reviews, case conferences and progress reports to referring/non-referring Physicians or primary health care providers
- Perform other duties that support the mission and vision of the organization and/or as assigned based on position responsibilities and/or site requirements.

- Minimum of 3-5 years of nursing experience in a community health setting or combination of community and hospital or public health settings
- Experience working effectively with communities and individuals who face barriers accessing traditional health care institutions, experience with clients with mental health and addictions
- Previous experience in a Primary Care setting preferred
- Phlebotomy skills, including experience with venipuncture and blood collection, are considered an asset.
- Experience with immunizations and vaccine administration
- Demonstrated ability to be flexible and possess strong organizational skills
- Strong attention to patient and family-centered care and establishing therapeutic relationships is required
- Demonstrated ability to work in an inter-professional team environment is required
- Experience with electronic health care records; experience using PS Suites an asset
- Strong communication and excellent interpersonal skills, combined with good analytical, problem solving, organizational and administrative skills
- Clean driver’s abstract, current, valid Class “G” Driver’s License and reliable transportation to support travel across Windsor-Essex County as required
- Bilingual (English, French, Arabic, or Spanish) language skills an asset.
- Proficiency in the use of computers and various software applications is required.
- Diploma in Practical Nursing from a recognized College or School of Nursing
- Member of the Registered Practical Nurses Association of Ontario (RPANO) preferred
- Current Basic Rescuer (C) Certificate recognized by the Heart and Stroke Foundation of Ontario required
- Completion of a Police Clearance and Vulnerable Sector Check prior to commencing employment.
