Summary

The Director of Post Acute Services provides strategic leadership and operational oversight for the ACO’s network of skilled nursing facilities (SNFs) and other institutional post-acute providers. This role drives performance across quality, cost, and patient experience metrics while ensuring seamless care transitions and strong alignment with value-based care objectives. The Director partners with physicians, hospitals, and SNF leadership to implement evidence-based care pathways, monitor network adequacy, and achieve shared savings goals.

Key Responsibilities

  • Lead strategy, contracting, and performance management of preferred SNF partners across the ACO network.
  • Establish and track KPIs such as length of stay (LOS), readmission rates, cost per member per month (PMPM), and patient satisfaction.
  • Develop and implement programs to improve care coordination between hospitals, SNFs, and primary care providers.
  • Monitor referral patterns, bed availability, and geographic coverage to ensure network adequacy.
  • Provide actionable data and insights to leadership to inform network expansion and performance improvement.
  • Ensure compliance with CMS requirements and all regulatory standards for skilled nursing care.
  • Represent the organization in contract negotiations, community meetings, and professional conferences related to skilled nursing.
  • Support education initiatives for providers and care teams on best practices for post-acute care and transitions.
  • Assist in organizational growth by initiating strategic relationships with skilled nursing facilities nationwide.

Supervisory Responsibilities

Directly supervises Post-Acute Care Managers, Network Analysts, and other assigned staff supporting skilled nursing initiatives. Provides mentorship, performance feedback, and professional development opportunities.

Qualifications

Education & Experience

  • Bachelor’s degree in Healthcare Administration, Nursing, Public Health, or related field required; Master’s preferred.
  • Minimum 8–10 years of progressive experience in skilled nursing operations, post-acute care management, or value-based care, with at least 5 years in a leadership role.
  • Demonstrated success managing SNF performance metrics and regulatory compliance.

Skills & Competencies

  • Strong leadership and team management skills, with the ability to mentor and develop staff.
  • Excellent consulting and relationship management skills, with a proven ability to engage specialist physicians, executive stakeholders, and practice leaders.
  • Advanced project management expertise, including portfolio management and strategic planning.
  • In-depth knowledge of skilled nursing operations, CMS regulations, physician practice operations, and value-based care models.
  • Strong financial and analytical skills with the ability to interpret data, develop insights, and present recommendations.
  • Highly effective communication and presentation skills for internal and external audiences, including executive leadership and providers.
  • Ability to manage change and drive results in a complex, fast-paced environment.

Collaboration & Education

  • Partner with Provider Consultants to deliver SNF updates, performance metrics, and analytics.
  • Educate PCPs on plan of care reviews, including key components and documentation requirements.
  • Facilitate discussions with providers on appropriate levels of care, reducing avoidable admissions, and improving continuity.
Operations & Compliance
  • Monitor and report on SNF performance metrics (LOS, readmissions, PMPM, preferred network utilization).
  • Ensure adherence to CMS regulations and maintain current knowledge of federal rulings impacting post-acute care.
  • Represent the post-acute network development team in All Member meetings, POD meetings, and off-site engagements as needed.
