Brief Overview
- Acts as a resident advocate, promoting the welfare and dignity of all residents.
- Ensures that documentation supporting resident care is maintained and updated as required.
- Facilitates the care planning process for residents within the guidelines of state regulations.
- Recognizes that families are an important support for resident care and involves them in the overall plan of care.
- Communicates effectively with family members regarding resident's condition, preferences and needs and addresses concerns in a respectful manner.
- Monitors resident's condition appropriately responds to changes in condition; participates in the delivery of quality care and services.
- Coordinates resident care and services using sound managerial and clinical judgment, appropriate objectives and high standards of care and customer service.
- Maintains effective communication with staff, peers and administrative personnel and with particular attention focused on neighborhood teams.
