Job Overview

The Community Health Coordinator serves as a dedicated liaison to the Hispanic community, driving health equity through strategic outreach, program development, and advocacy. Operating within the Community Health department, this professional leads and coordinates key culturally tailored initiatives, including the El Poder Sobre La Diabetes and Senos Sanos programs, while facilitating critical community forums like the Latino Advisory Council. The role focuses on delivering linguistically appropriate health education, breaking down social determinants of health, and representing the hospital at internal and external events to ensure the Hispanic population has trusted access to preventative screenings and clinical resources.

Qualifications

Required:

  • Undergraduate degree (Bachelor's Degree) in a related field.
  • Bilingual Fluency: Must be fully bilingual in English and Spanish with the ability to communicate, present, and create marketing materials effectively in both languages.
  • Strong cultural competence with a deep understanding of health disparities, barriers to care, and specific needs within the Hispanic community.
  • Excellent public speaking, relationship-building, and organizational skills to lead councils and presentations.
  • Flexibility to work non-regular working hours, including evenings (after 5:00 PM) and weekends, to support community functions and health fairs.

Preferred:

  • Three (3) or more years of related job experience in community outreach, public health education, or healthcare program coordination preferred.
  • Experience navigating grant compliance, tracking clinical outcomes, and drafting program reports.

Scheduling Requirements

  • Shift: Day Shift (with mandatory evening and weekend variations based on event schedules)
  • Status: Full-Time (FT)

Essential Functions

  • Hispanic Outreach Program Management: Manages and executes the El Poder Sobre La Diabetes and Senos Sanos programs by scheduling courses, registering participants, building curriculum alongside dietitians, navigating individuals to clinical services, and ensuring total grant compliance.
  • Culturally Appropriate Marketing: Promotes community health initiatives and programs using linguistically and culturally accurate marketing, educational handouts, and campaign materials.
  • Strategic Health Campaigns: Develops target outreach campaigns for core hospital preventative services, including mammograms, lung screenings, diabetes care, and colonoscopies.
  • Latino Advisory Council & Partnerships: Coordinates all logistics for the Latino Advisory Council meetings and actively builds sustainable partnerships with local community-based organizations.
  • Community Education & Presentations: Educates community groups on health-related topics and hospital resources; designs and delivers professional presentations and manages a dedicated Hispanic Speakers Bureau.
  • Screening Coordination & Tracking: Collaborates with the Community Benefits Specialist to identify eligible individuals for preventative care; issues linguistically appropriate screening reminders via mail or MyChart and tracks outreach data for regulatory reporting.
  • Social Impact (SICI) Support: Elevates health equity by collaborating with the Healing Homes Social Worker to support initiatives addressing food insecurity, housing, workforce development, and access to care.
  • Event Representation: Acts as the primary hospital liaison and professional representative at local health fairs, exhibits, and community functions, working with the Outreach Specialist to maintain high-quality table exhibits.
