SUMMARY
To support the overall implementation of quality of programs and services in accordance with all federal, state and corporate standards and regulations. This position will support the planning of client service. The overall implementation of quality programs will be monitored to ensure that services are being provided to clients in the group home and in accordance with all federal, state and corporate standards/regulations. To provide support to Program Managers and residential program.
ESSENTIAL FUNCTIONS
• Advocate for all client needs and welfare
• Train and assist Program Managers with implementation of the Individual Support Plan (ISP) process including but not limited to, tracking and training on client’s program objectives, client monthly reviews, client Interdisciplinary Team meetings and clients ISP documentation.
• Train in-coming staff (orientation) on Programmatic Topics.
• Assist Program Managers in providing training to DSP’s on how to teach specific goals identified in the clients ISP, as needed.
• Complete validity check of programs as a quality measurement to assist the Program Managers.
• Complete home visits during training time to validate programs are being accurately completed.
• Assist with monitoring medical appointment progress notes for any necessary follow-up and/or IDT’s.
• Monitor the timely implementation of ISP’s, and all client specific documentation associated with them including but not limited to: ISP, BSP, Quick Fact Sheet, Formal Programs, Meal Plans, etc.
• Monitor and track appropriate participation and representation in ISP and IDT meetings.
• Assist PM in monitoring to ensure that COS decisions are followed through in a timely manner by monitoring IDT and ISP Action steps.
• Participate in Incident Management meetings. Assist with note taking and monitoring to ensure timely and detailed follow-up notes related to incident Management.
• Provide assistance to the PM in tracking and trending the success of programmatic goals.
• Coordinate and provide oversight of the Human Rights Committee by attending/facilitating the Human Rights Committee meetings and actively assisting Program Managers to identify rights restrictions and develop effective replacement goals of said restrictions as appropriate.
• Assist with Tracking Restriction Review Dates in order to maintain compliance as well as track the number and type of restrictions as a quality measurement.
• Act as role model by maintaining and demonstrating professional in-service competencies. Actively promote and contribute to employee in-service training.
• Assist with maintaining appropriate positive contact with families and conservators all clients.
• Ensure rules and regulations relative to client’s program, health care and safety are followed.
• Participate in monthly Safety Committee meetings as Programming liaison.
• Attend ISP and Circle of Support (COS) meetings as necessary. Assist in the development of appropriate treatment strategies. When possible, assist PM with detailed note taking and follow-up.
• Assist in monitoring Program Target Dates and client program progression.
• Attend House Meetings as assigned.
• Complete internal surveys on a regular basis including but not limited to, chart audits, program reviews and observations as outlined by the Program Services Director.
• Participate in the development, monitor the facilitation and completion of Plan of Corrections related to DIDD Survey’s and internal CTE’s.
• Perform Program Manager duties during the interim of a vacant PM position, the absence of a Program Manager (due to illness or vacation), or as assigned by the Program Services Director. In this instance, assigned job responsibilities will be reviewed and altered as appropriate by the Program Services Director.
• Provide ongoing training and support to the Program Managers and other members of the OAC team as appropriate.
• Ensure appropriate financial considerations are adhered to, such as review of quarterly financial statements, burial plans, Contact of parents or conservators for over $100.
• Act as a liaison to OAC contract and employed Consultants (PT, OT, SLP, LBA, RD) as needed.
• Establish a positive rapport with each discipline (Residential, Nursing, Consultants, Medical Administration) to aid in the creation of client centered plans.
• Participate in new client admissions via home visits, facility tours, family/client interviews, intake meetings and staff training.
• Participate in Administrator On Duty procedures as outlined by OAC protocols.
• Work, participate in, and lead teams and committees as assigned.
• Provide support and assistance to the QSD in preparation for, during, and follow up resulting from IMC meetings as needed.
• Assist with maintaining appropriate positive contact with families and conservators of all clients as needed.
• Assist as a facilitator for psychiatric and behavioral reviews under the guidance and direction of an LBA.
• Complete all other duties as assigned.
QUALIFICATIONS
Education & Experience:
- Bachelor’s degree in the human service discipline.
- 3 years of experience working in the ID/DD/Behavioral Health field.
- Must be QIDP qualified
- Considerable knowledge in Medicaid/Licensure regulations
- Effective Time Management/Organizational skills
- Valid drivers’ license required
- CPR certification required within 30 days of hire
- Certified in Professional Crisis Management (PCM) within 60 days of hire is required
Competencies:
- Must have strong case management credentials
- Proficient in policy/procedures, protocols
