Therapist- Psychosocial Support

Location : Newport Beach, CA 92660
Schedule: Monday–Friday, 8:30 AM–5:00 PM
Compensation: $59.00 per hour
Candidate Requirement: Local candidates only
Setting: Hospice / Home-Based Patient Care / Community Health

We are seeking an experienced Therapist MS to provide comprehensive psychosocial assessments, care coordination, treatment planning, and ongoing support for patients and families. This role supports patients experiencing adjustment to illness, grief, hospice-related needs, addiction, crisis situations, and other psychosocial stressors.

• Conduct comprehensive biopsychosocial assessments.
• Evaluate patient and family needs related to illness, grief, hospice care, addiction, adjustment, and psychosocial stressors.
• Develop and implement individualized psychosocial treatment plans.
• Create discharge and termination-of-treatment plans.

• Bachelor’s degree required.
• Hospice experience required.
• Local candidates only.
• Experience with clinical assessments required.
• Experience with crisis intervention required.
• Experience working with culturally diverse patient populations required.
• Acute care experience required for certain departments.

Community Health

• Master’s degree in Social Work MSW or Marriage and Family Therapy MFT from an accredited school required.

Hoag Orthopedic Institute

• Master’s degree in Social Work MSW required.

• Experience in clinical assessments and crisis intervention in a hospital, acute care, hospice, or home-based setting.
• Bilingual or multilingual ability.
• Experience facilitating support groups or workshops.
• Experience precepting MSW or MFT interns.
• Minimum of 2 years of MSW experience preferred.
