Position Summary:
As the FSPT Coordinator, you'll serve as the central point of care coordination for children and adolescents with complex behavioral health needs. You'll facilitate multidisciplinary staffings, guide families through available services, coordinate individualized care plans, and ensure services are delivered in the least restrictive environment possible.
In this role, you will:
- Coordinate and facilitate regular FSPT and Child Family Staffing (CFS) with families, providers, schools, and community partners.
- Manage referrals from intake through eligibility determination and scheduling.
- Ensure referral packets and supporting documentation are complete and compliant.
- Partner with families to develop individualized service plans focused on preventing residential placement and supporting successful community-based care.
- Guide families through the Purchase of Services (POS) process and coordinate required approvals.
- Review and submit requests for non-traditional therapeutic support while ensuring compliance with funding requirements.
- Monitor service delivery, document outcomes, and coordinate ongoing staffing reviews.

Education and Experience Requirements for Behavioral Health Case Manager:
- Bachelor’s degree in the field of social work, criminal justice, psychology or special education is required.
- A minimum of one year’s experience working with youth in the behavioral health field.
- Strong organizational, communication, and relationship-building skills.
- Ability to coordinate multiple priorities while meeting deadlines.
- Experience using electronic health records and Microsoft Office applications.
- Commitment to family-driven, youth-guided, culturally responsive care.

Preferred Education and Experience for Behavioral Health Case Manager:
- Multiple years of experience navigating community resources and services in Northeast Florida.
