Job Description
SCMHCC is currently recruiting for a full-time Wraparound Facilitator. Benefits include 401(k) participation, 401(k) 5% salary match after one year of employment, eligible for paid vacation after six months of employment, paid sick leave accrual begins at hire date, employer-paid life insurance and LTD after one year of employment, no cost/low cost employee health insurance options, no-cost employee dental and vision coverage, and at least 10 paid holidays per calendar year.
$1,000.00 sign-on bonus option.
Job Summary:
SCMHCC is seeking a compassionate and organized Wraparound Facilitator for our Community Based Services (CBS) program. This provider supports youth and their families by helping them access community‑based mental health and support services. In this role, you will build strong relationships with families, collaborate with providers and community partners, and help ensure services are coordinated, effective, and responsive to each youth’s needs.
The ideal candidate is family‑focused, detail‑oriented, and comfortable working independently while staying connected with a multidisciplinary care team. This position offers the opportunity to make a meaningful impact by advocating for children, supporting families, and helping youth build skills for long‑term stability and success.
Position Responsibilities:
Minimum Qualifications:
Have at least a BA/BS degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substitution for one year of education. Must have the ability to communicate positively and effectively with others, and to work independently as well as be a productive part of a team. Must have a dependable vehicle and valid driver’s license. Must provide proof of insurance. Pass a criminal background and driver’s license check to the satisfaction of SCMH.
