Job Summary
Performs assessments of patient activity needs, develops a plan of care based on the assessment and implements the plan in conjunction with the patient and/or family. Participates as part of the interdisciplinary team.

Education

High School Diploma, GED, or equivalent
Experience:

Required

Completed two years in a social or recreational program within the last five years, one of which was full-time in a patient activities program in a health care setting or has completed an activity training course approved by any state.
Certification/Licensure:

Activities Director Certification

Must obtain within one (1) year of start date

Basic Life Support (BLS) Certification

Must obtain within thirty (30) days of start date

Safe Approach Level III Certification

Must obtain within six (6) months of start date
