Job Description

The Care Coordination Assistant is responsible for collaboration with the Care Coordination staff to assist with meeting the department and organizational goals. The daily functions include but are not limited to delivery and tracking of the “Important Message from Medicare” to Medicare beneficiaries who are in the hospital as inpatient status and documenting that this was done,, responsible for daily maintenance and updating of the Care Coordination databases, Navihealth referrals, Soarian documentation, communication with community health care providers as indicated for patient care transition. Provide back-up to the Care Coordination administrative services and duties. Assist the Director and Care Coordination staff with other tasks as needed.

REQUIREMENTS:

  • MINIMUM EDUCATION / CERTIFICATION / LICENSURE
    • Required:
      • High school diploma or equivalent required.
    • Preferred:
      • Associates degree preferred.
  • EXPERIENCE
    • Required:
      • Two (2) to three (3) years experience in Health Information Management (HIM), Business Office/Billing, Patient Access, or Case Management is required.
    • Preferred:
      • None specified.
  • SPECIAL SKILLS/EQUIPMENT

Must have the ability to manage time effectively and communicate in a professional manner. Must be able to use Microsoft Office programs including Outlook and a variety of department specific databases and systems (Soarian). Must be able to multi-task and remain flexible in a continuously changing environment. Must have the ability to type a minimum of 45 adjusted words per minute. Typing Test with be administered by Human Resources.
