Job Summary

We are seeking a detail-oriented and organized Intake Coordinator / Scheduler to join our home health team. In this role, you will be responsible for managing patient referrals, coordinating schedules for field staff, and ensuring smooth communication between clinicians, patients, and the office.

Responsibilities

• Manage incoming patient referrals and enter information into EMR (Axxess)
• Verify insurance, physician orders, and required documentation
• Coordinate and schedule patient visits with clinical staff
• Maintain accurate patient records, schedules, and logs
• Communicate schedule changes with field staff in a timely manner
• Perform welcome calls and assist with patient onboarding
• Track referrals, admissions, and visit completion
• Collaborate with clinical and administrative teams to ensure quality patient care

Qualifications

• High school diploma or equivalent required
• LPN/RN license preferred
• Home health or healthcare experience preferred
• Strong organizational and multitasking skills
• Excellent communication and phone etiquette
• Proficient in computer systems (EMR experience a plus)
