What you will do as an RN Case Manager:

  • Provide case management and coordinate care for assigned patients and families
  • Complete comprehensive assessments, admissions, and ongoing evaluations of patient needs
  • Collaborate with the Interdisciplinary Team (IDT) to develop and implement individualized Plans of Care
  • Deliver skilled nursing care including symptom management, IV therapy, wound care, and physical assessments
  • Educate patients and caregivers on medications, pain management, and daily care needs
  • Monitor patient condition and communicate changes to physicians and care team members
  • Maintain accurate, timely documentation in compliance with hospice policies and regulations
  • Provide emotional support and guidance to patients and families
  • Participate in IDT meetings, on-call rotation, and quality improvement activities

What we are looking for:

  • Graduate of an accredited nursing program with an active Virginia RN license
  • Minimum of 2 years nursing experience (hospice/home health or acute care preferred)
  • Comfort with EMR and technology
  • Reliable transportation with valid driver’s license and insurance
  • Compassionate, mission-driven, and emotionally resilient
  • A heart for mission-driven, relational work with individuals and families

Schedule & Compensation:

Schedule: Flexible hours around a core schedule (Monday-Friday approx. 8:30 AM–5:00 PM).

Starting Salary: $75,000/year

Benefits: 21 days of PTO in your first year, $.65 per mile mileage reimbursement, company paid life insurance and long-term disability insurance, medical through an ICHRA model offering multiple plan choices, and 401(k) with company match (100% match on the first 3%, 50% match on the next 1%)
