Psychiatric Mental Health Nurse Practitioner – PMHNP | CMH Behavioral Health Transitional Clinic at Citizens Memorial – Bolivar, Missouri

Citizens Memorial Hospital is expanding behavioral health access in rural Missouri and is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide care in Behavioral Health Transitional Clinic housed in Bolivar Family Care Center in Bolivar, MO.

This clinic is designed to provide timely support for patients who need behavioral health care while waiting to establish with a long-term CMH behavioral health provider. It is a bridge model of care focused on stabilization, support, and continuity.

Position Highlights

• Full-time position
• Monday through Friday
• Clinic hours: 8:00 a.m. to 5:00 p.m.
• No evenings, no weekends, no call

What You’ll Do

You will assess, diagnose, and treat mental health conditions for patients ages 6 and up referred by a medical provider for timely behavioral health support.

Key responsibilities include:

• Psychiatric evaluation and medication management
• Psychotherapy and therapeutic support
• Development of comprehensive treatment plans
• Patient and family education
• Coordination of care with other healthcare providers
• Supporting patients until they transition to an ongoing behavioral health provider within CMH

This is not a traditional long-term caseload. It is a transitional care role that requires clinical versatility, efficiency, strong rapport-building skills, and effective handoff communication.

Qualifications

• PMHNP-BC certification required or ability to obtain
• Missouri licensure required or ability to obtain
• Comfort treating a broad patient population, including pediatric patients age 6+
• Ability to work efficiently in a fast-paced, team-based setting
