Job Overview
We are seeking a dedicated and compassionate Home Health Nurse to provide high-quality nursing care to patients in their homes. The ideal candidate will possess a strong understanding of patient care standards and be skilled in managing various medical conditions. This role requires the ability to work independently while collaborating with healthcare teams to ensure optimal patient outcomes. The Home Health Nurse will be responsible for delivering personalized care, monitoring patient progress, and providing education to patients and their families.
Responsibilities
- Conduct comprehensive assessments of patients' health status and needs.
- Develop and implement individualized care plans based on assessments.
- Administer medications and treatments as prescribed, ensuring adherence to standards.
- Monitor vital signs and report any changes in patient condition to the healthcare team.
- Provide wound care, including dressing changes and infection prevention.
- Educate patients and families on disease management, medication administration, and home care techniques.
- Collaborate with multidisciplinary teams, including physicians, therapists, and social workers.
- Maintain accurate documentation of patient care activities in compliance with regulations.
- Ensure compliance with workers' compensation law and relevant healthcare standards.
Experience Candidates should have a background in nursing with experience in critical care settings such as hospitals or Level II Trauma Centers. Familiarity with ambulatory surgery centers is preferred. Knowledge of ICD-10 coding is a plus. Experience in specialized areas such as PICU or toddler care will be beneficial. The ideal candidate should demonstrate proficiency in administering treatments like Botulinum toxin treatment and possess a strong understanding of critical care protocols. A commitment to ongoing professional development is essential for success in this role.
