About This Position

Social Worker – $45,760/year

What You’ll Do:

  • Conduct psychosocial assessments and provide counseling to residents and families
  • Support admissions, transitions, and adjustment to aging or illness
  • Collaborate with the interdisciplinary team on care planning and documentation (including MDS)
  • Assist with discharge planning and coordination of community resources
  • Advocate for residents and provide crisis intervention as needed
  • Maintain accurate documentation and support financial/insurance processes
  • Serve as a liaison with families, agencies, and mental health services

What You’ll Need:

  • Licensed Social Worker (per state requirements)
  • At least 1 year of interdisciplinary team experience, with long-term care or hospice preferred
  • Strong communication, critical thinking, and documentation skills
  • Ability to work in a fast-paced, emotionally sensitive environment
  • Proficiency with computer systems and maintaining confidentiality
  • Ability to collaborate effectively with residents, families, and care teams
