Position Summary:
The Hospice Home Health Aide provides compassionate personal care and supportive services to terminally ill patients in their homes or residential facilities. Working under the supervision of a registered nurse or other licensed clinician, the aide helps ensure patient comfort and dignity during end-of-life care.
Requirements: Key Responsibilities:- Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility.
- Provide companionship and emotional support to patients and families.
- Take and record vital signs such as temperature, pulse, respiration, and blood pressure as instructed.
- Support nutritional needs by assisting with feeding, meal prep, or monitoring intake.
- Perform light housekeeping related to patient care, such as changing linens or cleaning personal areas.
- Observe and report changes in the patient’s condition or behavior to the supervising nurse.
- Follow the individualized care plan developed by the hospice team.
- Use proper infection control and safety techniques at all times.
- Participate in ongoing training and team meetings as required.
- High School Diploma or GED.
- Certified Home Health Aide (CHHA) or Certified Nursing Assistant (CNA) with current state certification.
- Minimum [6 months – 1 year] experience in home health, hospice, or long-term care preferred.
- Valid driver’s license and reliable transportation (if travel is required).
- Ability to work independently and communicate effectively with patients, families, and interdisciplinary team members.
- Empathy, patience, and a deep respect for the dignity and needs of terminally ill patients.
- Involves travel to patient homes.
- Exposure to emotional and physical demands associated with end-of-life care.
- Must be able to lift or assist in lifting patients when necessary.
