We are a newly established hospice organization committed to delivering compassionate, dignified, and personalized end-of-life care to patients and their families in the Milwaukee area. As we grow, we're seeking a dedicated HHA to join our clinical team and help shape the future of our care model.

Position Summary:

The Hospice Home Health Aide provides compassionate personal care and supportive services to terminally ill patients in their homes or residential facilities. Working under the supervision of a registered nurse or other licensed clinician, the aide helps ensure patient comfort and dignity during end-of-life care.

Requirements: Key Responsibilities:
  • Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility.
  • Provide companionship and emotional support to patients and families.
  • Take and record vital signs such as temperature, pulse, respiration, and blood pressure as instructed.
  • Support nutritional needs by assisting with feeding, meal prep, or monitoring intake.
  • Perform light housekeeping related to patient care, such as changing linens or cleaning personal areas.
  • Observe and report changes in the patient’s condition or behavior to the supervising nurse.
  • Follow the individualized care plan developed by the hospice team.
  • Use proper infection control and safety techniques at all times.
  • Participate in ongoing training and team meetings as required.
Qualifications:
  • High School Diploma or GED.
  • Certified Home Health Aide (CHHA) or Certified Nursing Assistant (CNA) with current state certification.
  • Minimum [6 months – 1 year] experience in home health, hospice, or long-term care preferred.
  • Valid driver’s license and reliable transportation (if travel is required).
  • Ability to work independently and communicate effectively with patients, families, and interdisciplinary team members.
  • Empathy, patience, and a deep respect for the dignity and needs of terminally ill patients.
Working Conditions:
  • Involves travel to patient homes.
  • Exposure to emotional and physical demands associated with end-of-life care.
  • Must be able to lift or assist in lifting patients when necessary.
