Palliative Care Navigator in Statesville, North Carolina at Hospice and Palliative Care of Iredell County

We are seeking a Palliative Care Navigator, with a focus on principal care management. This is a full-time (40 hour) position, Monday through Friday. The palliative care navigator is responsible for enrolling patients in the care management program, developing individualized care plans, conducting follow-ups to monitor progress and address needs, and overseeing patient engagement and program participation to ensure continuity of care and achievement of health goals.

• Active Certified Medical Assistant (CMA) certification or willing to obtain.
• Experience in a clinical office setting, palliative & hospice care preferred.
• Computer skills are required.
