Join Choice Health at Home as a Physical Therapist and provide personalized care that makes a meaningful difference in patients’ daily lives. You’ll assess patient needs, develop individualized treatment plans, and deliver therapy that improves mobility, reduces pain, builds strength, and restores function.

As a Physical Therapist, you will:

• Perform and document initial evaluations to identify functional limitations and establish appropriate treatment plans.

• Develop and implement individualized therapy programs to improve movement, restore physical function, relieve pain, and prevent further decline.

• Monitor patient progress and adjust treatment plans to achieve the best possible outcomes.

• Educate patients and caregivers on exercises, safe mobility, and strategies to support independence.

• Collaborate with patients, families, and healthcare team members to ensure continuity of care.

• Follow infection control practices and universal precautions during patient care.

• Complete accurate documentation within company-established timelines.

• Support agency policies, procedures, and commitment to quality care.

Requirements

• Graduation from an accredited physical therapy program and successful completion of the state licensure examination.

• Current, valid Physical Therapy license.

• One year of home health experience preferred.

• Current CPR certification.

• Valid driver’s license and auto insurance.

• HomeCare HomeBase experience preferred.

Physical Demands

• Ability to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.

• Ability to lift, carry, pull, or push up to 100 pounds.
